Women Get A New Breast Augmentation Option: Silicone Implants

At a recent American Society of Plastic Surgeons meeting, it was announced that the FDA had approved silicone breast implants, and that soon, all plastic surgeons will be able to provide their patients with a new breast implant option.

At times, the announcement caused fever pitched conversation and a large buzz within the crowd. It seemed as though the announcement was like the Holy Grail was now being provided to plastic surgeons across the country. Why?

“The approval of silicone implants is a great thing, but the choice between the two is something the patient needs to decide, not the plastic surgeon,” says Dr. Fata in Indianapolis, Indiana.

Since 1992, when the FDA banned silicone implants, breast augmentation surgeries have grown 306%. Clearly, not having a silicone breast implant option is not that big an issue.

There are two questions that must be answered: First, Are saline implants really that bad? Second, What are the benefits (or possible benefits) of silicone implants?

To answer the first question, no. “Saline implants are a very good option for women who want to enhance their appearance,” says Dr. Fata “Obviously, if saline implants were an inferior product, the number of breast augmentation procedures would not have sky rocketed in the past 13 years.”

The benefits of saline implants include:

High Approval Rate (Over 90% of breast augmentation patients would recommend the procedure to another woman)

Proven Track Record

Sterile saline solution is basically harmless to the body

The complication rate has dropped

Fewer number of ruptured implants

Fewer number of capsular contractures (formation of hard scar tissue)

Unfortunately, saline implants are not as soft as natural breasts, they are heavier, they can cause sagging over time, and sometimes they can cause noticeable ripples in the skin.

So what will silicone implants provide? At the moment, since they have not been widely used since 1992, there is not a proven track record that can provide us with all the benefits silicone implants can provide. So why are all of these plastic surgeons so excited about the approval of silicone implants?

The possible benefits of silicone implants include:

Softer than saline implants

Less chance of rippling

Less chance of capsular contracture

Lighter, and thus less sagging

At the moment, silicone implants are not being widely used, and because of this, we cannot tell if they will in fact be able to provide women with less chance of rupture, capsular contracture, or leaking into the body. What is known is that the new cohesive silicone implants are lighter and feel more natural.

Silicone implants are now being made out of a cohesive silicone solid gel. This means that the inside of a silicone implants should stick together, so that if the shell of the implant was to break or rupture, the gel inside would not leak into the body. This is a great advancement over the silicone oil that was used in older implants.

In saline implants, the sterile saline solution is harmless to the body, so if there was an implant rupture, then there was no harm done. With the older silicone oil implants, if there was an implant rupture, then the silicone oil used to fill the implant could create a number of different problems after entering the body.

So what should plastic surgeons do? The choice between silicone and saline implants should be the patients. At the moment, it isn’t really possible to tell which implant is better than the other. Plastic surgeons should provide their patients with both options and the available information about them; once that is done, the patient can determine which one is right for them.

There is no reason for breast implant patients not to try the newest generation silicone implants. However, they should be shown all available data, or lack there of, on both implants. The only thing missing from silicone breast implants is a track record of their advantages and disadvantages. Only time will be able to show what type of implant is better, or which implant is chosen more by patients.

Prostatitis Can Be Cured

Prostate infections are extremely difficult to diagnose and treat because many doctors do not order the right tests and even when they make the correct diagnosis, they often fail to treat the condition long enough or they fail to treat infected partners.

Symptoms: Chronic prostatitis is characterized by a feeling of having to urinate all the time, discomfort during urination, terrible discomfort when the bladder is full and having to get up many time each night to urinate. Benign prostatic hypertrophy or large prostate causes difficulty starting stream and dribbling. Prostate cancer rarely causes burning on urination or urgency.

Tests: Total and free PSA blood test to screen for cancer, check your prostate and sometimes order a sonogram of the prostate. If I am suspicious of a prostate cancer, I will refer you to a urologist for further evaluation. Your doctor can order a urinalysis and urine culture. If abnormal, your doctor will treat a urinary tract infection, but it is usually normal. Then, your doctor may do a special swab for chlamydia and gonorrhea and request a semen culture. No test is available for practicing physicians to diagnose mycoplasma or ureaplasma and the test for chlamydia is not dependable. The only dependable test for prostate infection is for your doctor to massage your prostate to collect secretions and immediately check your secretions for white blood cells. If a) he sees more than 5 white blood cells in your secretions under the microscope, or b) the white blood cells are clumped together; or c) your urine has a positive leukocyte esterase or nitrite test and your urine culture and semen do not grow a germ, the odds are overwhelming that you are infected with chlamydia, mycoplasma or ureaplasma or something else that cannot be diagnosed.

Failure to see white blood cells in your secretions does not rule out a prostate infection. Most urologists do not order semen cultures because they claim that they are always contaminated with germs from the skin. Recent studies from Cornell and Mexico City show that they are wrong (2,3). Most men collect semen cultures from the vagina or mouth which always is contaminated. If the patient is asked to shower and clean himself with soap and water, then collect the specimen with his hands and then let it squirt into the culture jar without touching it, the culture is very dependable (2).

Prostate infections are extraordinarily difficult to cure because antibiotics do not accumulate in high concentrations in the prostate. So most men have to take appropriate antibiotics for many months and their partners have to take the same antibiotics for a much shorter duration. I often prescribe a quinolone antibiotic called Tequin for several months, or a combination of Tequin and doxycycline. If you do not feel much better by six weeks, you may need to ejaculate frequently or find a kind urologist who will agree to massage your prostate at least once a week while you take the antibiotics. UCLA researchers report that 40 percent were cured, 20 percent were initially cured and then had a recurrence perhaps because the partner was not treated, 21 percent were improved and only 21 percent had no improvement. All the men who had bacteria in their semen cultures produced sterile specimens after treatment.

Treatment of prostatitis with long-term antibiotics is controversial and not accepted by many doctors; discuss this with your doctor.

What is Erectile Dysfunction? Erectile Dysfunction is a condition in which a man does not get an erection or may be unable to consummate a sexual intercourse. This may occur at any age but is more common in older men.

Emotional Problems: Negative feelings like nervousness due to some bad experience or a previous incident of impotence, work related stress or stress due to family affairs, depression or lack of confidence, a turbulent relationship with your partner can be one of the causes too.

A change in your lifestyle to make it less stressful and simple would definitely improve if not cure this condition. Tobacco IS NOT Just Another Consumer Product!

There are currently 1.1 – 1.4 billion smokers and tobacco users in the world and about 4.9 million of them will die THIS YEAR from smoking and tobacco related diseases. Tobacco IS NOT just another Consumer Product. This misconception would have to go down in history as one of the most successful product marketing shams. Why? How is it that a product can be marketed as giving flavour and satisfaction, when what you really get is addiction, sickness and poverty.

Tobacco is one of the few legal substances available in stores as a ‘consumer product’ that will kill or harm when used as intended. Wow! Forgive me if I sound a bit cynical towards the tobacco industry. Three thousand kids become new smokers every day. I’m more in shock than cynical!

I believe that anyone can quit smoking if they choose, even without the use of Nicotine Replacement Therapy products. This statement may seem like a slap in the face for the giant NRT companies, but I’m sorry it’s true. Let me make another statement, ‘Nicotine Addiction is the World’s Greatest Deception’ and every smoker and tobacco user falls for the same lie.

I have examined the link between the ‘emotional gradualisation’ and the physiological rewiring of the brain from nicotine. The gradualisation process for most smokers is the product of a gradual numbing of the emotions to the point where our responses are slowly hardened to the consequences of smoking related disease. Even when facing death. In short anti-smoking emotional fear tactics do not work effectively for all smokers. Do people really take any notice of cigarette packet warning labels? I didn’t!

The bottom line is this…tobacco does kill. Smokers and tobacco users need urgent help to break the chains of the nicotine addiction. Tobacco reform laws and increased tobacco taxes will undoubtedly impose a certain amount of tobacco control.

However this will inadvertently cause unnecessary emotional trauma for many smokers being forced to stop smoking without access to comprehensive educational programs. Educational programs are essential for smokers to LEARN how to overcome the many internal and external psychological influences associated with nicotine addiction.

The multi-million dollar NRT industry, which seems to be riding very nicely on the back of the trillion dollar tobacco market, seems to target smokers and tobacco users with a very narrow medical approach, and as Alan I. Leshner Ph.D. Director of the National Institute on Drug Abuse (NIDA) once quoted “Science has also shown that treating addiction with medications alone is not nearly as effective as when coupled with a behavioral approach.” [source NIDA]

The world is facing a major health epidemic as a result of smoking and tobacco related diseases. Over 450,000 people will die this year in the US alone from these diseases. These are very serious issues and my soap box plea is that tobacco is not ‘just another consumer product’, it is the source of the world’s greatest deception.

Mums and Gums: Cause of Miscarriage and Premature Births

After I had my first child, my dentist complained that I hadn’t been looking after my teeth and gums. I was sure that I hadn’t been doing anything different from usual. He then went on to complain that all pregnant women and new mothers neglected their teeth and gums. I believe that he was wrong when he suggested that pregnant women suddenly change their oral hygiene habits. But he was correct that the dramatic hormonal changes during pregnancy can greatly affect the gums. If the gums aren’t healthy to start with then this seemingly irrelevant problem can seriously baby’s growth and health in the womb.

It is now shown beyond any doubt that maternal gum disease causes premature labour. One study performed prospectively in North Carolina looked at the effect of dental health in 812 women. Each woman’s dental health was assessed at 26 weeks and then within 48 hours after delivery. The researchers examined both the rate of premature births (less than 28 weeks) and low birth weight (less than 1,000 gms). Their data was adjusted for race, parity and gender. 1.1% of the group with healthy gums (201 women) had premature births. This rate greatly increased in the 566 rated as having mild gum disease to 3.5% and was 11.1% in the 45 women with moderate to severe gum disease. The rates were slightly more dramatic for birth weight. No women with healthy gums had a baby that weighed less than 1,000 gm. Six per cent of the women with mild disease had a baby with low birth weight and 11.4% of those with moderate to severe disease had a low birth weight baby. Studies from Poland, France and Brazil showed similar results.

This research applies equally to second trimester and later miscarriages as well as prematurity at term.

Gum disease might cause low birth weight and prematurity in more than one way. Gum disease allows oral bacteria to move from the mouth into the blood stream. These oral bacteria can cause clotting and are known to be a major factor in strokes. They are also cause endocarditis, a potentially lethal infection of heart valves and inner lining of the heart. Periodontitis, serious gum disease in pregnancy might act through clotting or infection itself.

The great news is that early attention to periodontal (gum) health can reduce these risks.

A study from Chile examined the oral health of 400 pregnant women, aged between 18 and 35, before 28 weeks gestation. Dentists immediately treated periodontal disease in 200 of the women. They treated the other 200 (the controls) only after delivery. The rate of pre-term, low birth weight delivery was 1.8% (3/163) in the treated group and 10.1% (19/188) in the untreated group. Low birth weight and prematurity was 5.5 times more likely in the untreated group.

Prematurity and low birth weight together greatly increase the risk of a baby’s death. There are also a large number of other important problems that occur as a result of prematurity. It is sad that it has taken so long to recognise this easily preventable cause of problems.

Let us hope that doctors and dentists will soon work together to prevent the problems in the future. In the meantime, inform every pregnant woman that you know.

What Are the Benefits of Having a Foreskin?

I just don’t go into the reasons for circumcision, which change with every decade depending on whatever disease is in the spotlight. In the 1880s, circumcision was recommended to prevent insanity and epilepsy, in the 1940s it was recommended to prevent STIs, the 1950s it was a cure for cancer and in the early 21st century, HIV is spread in the moist regions of the foreskin as opposed to the dry, keratinised layer of the glans.

In my opinion, circumcision violates a major principle of medical practice: First, do no harm. It also violates all seven principles of medical ethics. Some doctors and nurses refuse to perform or assist with circumcisions because of ethical considerations

To make it clear I have no problem with circumcision as long as the person gives their full consent and is informed, as they are supposed to be with all procedures. The fact is when circumcision is performed, it does not treat any disease, injury, or other health problem. Since there is no urgency to do it, it must be delayed until the child is old enough to make the decision for himself.

Therefore, a male may make a decision to be circumcised when he is older without losing the benefit of having foreskin.

The foreskin is an integral, normal part of the penis. It contains about 240ft of nerves, and around 1000 nerve endings. This fact explains why anesthetics provide incomplete pain relief during circumcision. Without the coverage of a foreskin, the glans penis dries out and becomes keratinised (i.e. dry, thick, insensitive – think what would happen to the moisture surrounding the eyeball if the eyelid was removed) and takes on the function of the outer foreskin – protection from dirt, chafing and otherwise outside threats.

Without the foreskin, around 80% of the penis’ erogenous zones are lost, keratinisaton occurs (as I mentioned above) and the gliding action of the foreskin over the erect glans is lost, not to mention any risks associated with such surgery, including the formation of ’skin bridges’ where the foreskin reattaches itself to the glans, skin ‘tags’ where the foreskin was incompletely cut away, scarring and excess skin removal.

In a national survey, circumcised men reported they were more likely to engage in masturbation, heterosexual oral sex, and anal sex than intact men. The result suggests that circumcised men seek alternative forms of stimulation to compensate for reduced sensitivity.

The complex anatomy and function of the foreskin dictate that circumcision should be avoided or deferred until the person can make an informed decision as an adult.

When most people talk about plastic surgery they are usually discussing various issues from a female perspective. However today men undergoing plastic surgery for cosmetic reasons is becoming a much more common practice.

Most men having surgical procedures done are doing so for the same reasons as most women and that is to simply enhance their physical appearance. There are even a growing number of plastic surgeons who specialize primarily in male related cosmetic surgery.

The type of plastic surgery men are having performed are much the same as those that have already been common place to women over the years. Procedures to smooth away wrinkles and reduce the signs of aging, as well as body sculpting to create a more fit and healthy looking physique.

The resulting effect of this is a greater blood supply in the facial region which creates a higher risk of potential bleeding. This increased blood flow does have a note worthy positive affect in that most men heal better and slightly quicker than women when undergoing certain procedures.

It is imperative that anyone considering plastic surgery find a highly skilled and qualified cosmetic surgeon. Finding a surgeon who specializes in procedures for males would be the ideal situation even if you are undergoing a surgical procedure common to both genders. It is important to note that all surgery cosmetic and otherwise are very serious and life-changing experiences.

Make sure you seriously think about any potential surgeries you’re contemplating and are comfortable with the associated risks. Many surgeons have noticed men on the whole are less happy with the results of cosmetic surgery then are women even when everything looks well and has gone smoothly. The reason for this has a lot to do with the motivations behind the decision to get surgery to begin with.
